1. JAMB admission letter

2. FUOYE Admission Letter

3. Passports

4. SSCE/IJMB/A level Results

5. Birth Certificate

6. Completed Bio-data Form

7. Letter of Attestation (Duly signed by Parent)

8. Guarantor’s Letter (Duly signed by Clergy, Officers from grade level 13 and above)

9. Court Affidavit of Good Conduct

10. Medical Report (from University Health Centre)

Note the Following:

1. Locate your department to find out other requirements.
2. Make 3 Photocopies of each of the above-mentioned documents and get an office flat file for each of them.
3. Your O’LEVEL result must not be original.
4. After the submission of your first file, you will be given another file with forms to fill.
5. Red Background Passport recommendable.
